Sign your participant waiver.

I will be participating in the above-mentioned program or activity. I understand that this program or activity will not be supervised or taught by a City of Lewisville employee or agent. The instructor of the above-mentioned program or activity is an independent contractor, and the City of Lewisville is not responsible for the instructor's actions.

Assumption of risk, waiver of liability and photo release

In consideration of being allowed to participate in a City of Lewisville Parks & Recreation Department (PARD) program or activity, the undersigned hereby releases, waives, discharges and covenants not to sue, and agrees to hold harmless for any and all purposes, the City of Lewisville, Texas, its officers, employees, successors, assigns, sponsors and volunteers (collectively "the City") from any and all liabilities, loss, claims, demands, costs (including court costs and attorney's fees), personal injury (including exposure and illness from infectious diseases, both bacterial and viral (including but not limited to MRSA, SARS-CoV-2, COVID-19 and influenza), and death), or property damage that may be sustained by me while participating in any PARD program or activity.

I choose to voluntarily participate with full knowledge that said participation may be hazardous to me and my property. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including illness and death, which may be sustained by me as a result of participating in the PARD program or activity, regardless of whether the liabilities, loss, claims, demands, costs, personal injury or property damage sustained by me is as a result of the negligence, gross negligence or strict liability of the City.

I hereby give my permission to use my photographic or video imaging and pre-approved quotes on the PARD websites, social media outlets, brochures, and any other electronic or printed materials or to promote the PARD or the City of Lewisville.

I understand that I must execute this Assumption of Risk, Waiver of Liability and Photo Release in order to participate in a PARD program or activity.

I have carefully read this release and waiver of liability and fully understand its contents. I am aware that this is a waiver and release of liability and I sign it of my own free will.
